MEDICAL FORM REQUIREMENTS

Scouts: A completed medical form is essential for camp participation (no exceptions). A Class 1 form must be reviewed, updated and signed by a parent/guardian each year.
The Class 2 portion of the form which requires a doctor's evaluation must be completed every 3 years. To be current, your Class 2 portion must be dated July 6, 2005 or later.

Adults (< 40 years old) staying at camp < 3 days need a Class 1 form.
Adults (< 40 years old) staying at camp > 3 days need both Class 1 and Class 2 forms or a Class 3 form.
Adults (> 40 years old) staying at camp for any length of time need a Class 3 form no more than a year old.
